How do I create a QR code for a form in Microsoft Forms?

Use Microsoft Forms to easily create online surveys, forms, quizzes and questionnaires. Users can use the QR Code for print or digital display, as a link to the survey.

Instructions

1. Open office.com and sign in to your account.

2. Open the Microsoft Forms application from the app launcher.

3. Open the form that you will be using to collect data.

4. Click the Responses button from the top right navigation menu.

5. Select the QR Code icon from the sharing options pop-up.

6. Click the Download button located below the preview of the QR code.

7. Locate the downloaded QR code file from your machine's downloads folder and save it to a new location.
